Description
- A perforation of the wall of the small intestine or colon that results in intestinal contents contaminating the abdominal cavity. Perforations are divided into iatrogenic vs pathological vs traumatic (1).
- Perforations are divided by anatomy:
- Gastric/duodenal
- Small bowel
- Appendix
- Colon
Epidemiology
- Traumatic perforations (1):
- Small bowel:
- Injured in 80% of abdominal gunshot wounds
- Injured in 30% of abdominal stab wounds
- Injured in 5% of blunt abdominal trauma
- Colon:
- Injured in 20% of penetrating abdominal trauma
- Injured in 7% of transpelvic gunshot wounds
- Perforated duodenal ulcer:
- Incidence has decreased in recent long-term studies from 14 in 1,000 to 8 in 1,000 person years; perforated duodenal ulcer is 5% of all abdominal emergencies.
