BASICS
Description
Ovarian torsion is the twisting of the ovary on its ligamentous supports,
often resulting in the obstruction of its blood supply. It is more common
in the right ovary.
Epidemiology
In 1 hospital's 10-year review, ovarian torsion accounted for 2.7% of
surgical emergencies.
Risk Factors
- Pregnancy
- Ovarian hyperstimulation for infertility treatment
- Tubal ligation (intermittent partial torsion)
Pathophysiology
- Venous, arterial, and lymphatic flows are impeded due to the obstruction
of blood vessels in the ligament of the torsed ovary.
- First, because of the impedance of venous flow, the affected ovary
enlarges significantly, and subsequently ovarian ischemia can occur.
Etiology
- Ovarian cyst (48%)
- Tumor (46%)
- Approximately 90% are benign; however, in children < age 15,
80% are malignant.
